A contact page for you to use with your site, it records the user's ip, email, comments, user agent, and the url referer. It also lets them know if they have forgotton to fill in a feild and dosnt send the email till they go back and do. Uses php and javascript. Includes a required name, email address, subject and comments field along with extra web site, company, position, zip code, country, telephone, and fax fields and a unique message ID# for them, and you get there ip number, iphost, user agent, refer, site host (just to make sure that its not from any "bots") in your email. version 1.6 includes cleaned up code, fancy inputs (see demo/example), customizeable inputs, (you can choose if you want to show the website, company, position, postal address, zip code, country, telephone, and fax), and a javascript that only allows people to enter numbers in the zip code, telephone, and fax area. Furture Plans: adding a javascript that alerts people when then left the required fields blank, and make it even more customizeable
